in order to award you your hard-earned prize money.

FOR U.S. RESIDENTS:

Please download and print this Affidavit of Eligibility, Publicity Release and Liability Waiver. It's a legal document reaffirming your agreement to Adball's Terms & Conditions, Official Rules, and other policies. It also states that you are responsible for paying the applicable taxes on prize money you've won on Adball, and confirms that you're eligible to receive prize money in the first place.

Please print it, fill in all the required information, and mail it to us at:

We also need you to send us some IRS documents. Use our Tax Questionnaire to figure out which documents to send us. Then follow the links to download the appropriate documents, fill them out, and send them to the address above.

FOR NON-U.S. RESIDENTS

You may obtain 70% of your winnings by filling out the Affidavit of Eligibility, Publicity Release and Liability Waiver. Adball is required to submit 30% of your winnings to the US Government as withholding tax. If you wish to obtain the other 30% of your winnings please contact a Tax Consultant and the US Government. Laws vary by country.  Adball does not provide tax advice.

The sooner you return these documents to us, the sooner you can get your  cash, so download them, print them, fill them out, and get them in the mail ASAP!
